List and filters

Filter by driver, status (draft or submitted), accident date range, injuries, and DOT recordable.

Drafts

Open Continue draft to use the same seven-step wizard as the driver portal. On step 2, use Driver account to choose which driver-role login the accident belongs to (search by name or email). This is the app user, not a Samsara/Alvys import row. Changing the account reloads prefilled identity and vehicle fields when fleet data exists. Save drafts or submit on behalf of the driver (submitted_by records your user). List filters and per-driver counts use this login, not the free-text name on the form.

Submitted reports

View the full read-only document, Print, or Download PDF.

Delete

Delete report removes the database row and all uploaded photos, sketch, and signature files. Admins may delete drafts or submitted reports. This cannot be undone.
