Accident reports

Use Operations > Accidents & Incidents to find, review, and help complete driver accident reports.

Report list and filters

Search or filter reports by driver, draft or submitted status, accident date, injuries, and DOT-recordable status. Select a report to open it.

Draft reports

Select Continue draft to open the seven-step accident form. You can review the driver, accident details, vehicles, sketch, photos, and signature before saving or submitting.

On the driver step, Driver account controls whose record the report belongs to. If you change the driver, check all prefilled identity and vehicle information before continuing.

Submitted reports

Submitted reports are read-only. Use:

• Print for a paper copy.
• Download PDF to save or email the completed report.

Delete report

Only use Delete report when the report and all of its uploaded files should be removed. This action cannot be undone. If a submitted report must remain for safety, insurance, or legal records, do not delete it.
